The New FormAlign Prescription Form – Now available on the website!

FormAlign have been working away these past couple of months! After feedback from customers, we’ve built an online prescription form to make things even easier.  A list of our component-based seating products can now be put together, with our new online selection tool, by using the product images or codes via the search bar, to identify the specific components required to build the solution and outcome for your wheelchair user.  If you need to send this to your procurement team or simply send it to us for a quotation request, each step has been made easy and efficient.
